I acknowledge that by signing this Release, I have been given the full opportunity to ask any questions regarding my tattoo at Skin Graphics Tattoo & Piercing Studio, and all my inquiries have been to my satisfaction. I specifically acknowledge that I have been informed of the following facts and matters, and I agree the terms below:

I am not pregnant or nursing. I have disclosed any medical conditions such as Seizures (e.g., epilepsy), fainting, hemorrhaging (Blood loss), HIV-AIDS, or any communicable diseases, as well as heart conditions or medications that thin the blood. I have informed my tattooist of any conditions like diabetes that may affect the healing of the tattoo.

If I have Hepatitis A, B, or C, or any other communicable disease, I have notified the tattooist and have been informed of necessary procedures to ensure proper healing of my tattoo.

If i have any type of allergic reactions, infection or rash anywhere on my body, i will advise my tattooist.

I do not have any medical or conditions including, but not limited to, acne, scarring (keloid), eczema, contagious skin infections (impetigo), genital warts (if relevant to the requested tattoo), freckles, moles or sunburn in the area to be tattooed that may interfere with said tattoo. psoriasis at the tattooing site, or any open wounds or lesions at the tattooing location.​​
I have informed the tattooist of any allergies I have related to metals, latex, gloves, anesthetics, soaps, medications. I recognize that it is not feasible for the tattooist to assess whether I may have an allergic reaction to the tattoo or its associated processes, and I understand that such reactions are possible.
If have experienced any of the mentioned allergies, it is advisable to consult a GP before proceeding with the piercing, and written authorization will be required.

I acknowledge it is not reasonably possible for the representatives of Skin Graphics Tattoo & Piercing Studio to determine whether I might have an allergic reaction to the pigments or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible.
I acknowledge that infection is always possible due to improper hygiene as a result of the obtaining of a tattoo, particularly in the event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touch-up work needed, due to my own negligence, will be done at my own expense.

I realize that variations in colour and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin colour is dark, the colours will not appear as bright as they do on light skin.
I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo.
I acknowledge that a tattoo is permanent change to my appearance and that no representations have been made to me as to ability to later change or remove my tattoo. To my knowledge, I do not have physical, mental or medical impairment or disability which might affect my well being as a direct or indirect result of my decision to have a tattoo done at this time.
I acknowledge I am over the age of 18 years and that I have truthfully represented to my tattooist that the obtaining of a tattoo is by my choice alone. I consent to the application of the tattoo and to any actions or conduct of the representatives of Skin Graphics Tattoo & Piercing Studio reasonably necessary to perform the tattoo procedure.
I understand I will be tattooed using appropriate instruments and sterilization.​​​
