I acknowledge that by signing this Release, I have been given the full opportunity to ask any questions regarding my piercing at Skin Graphics Tattoo & Body Piercing Studio, and all my inquiries have been to my satisfaction. I specifically acknowledge that I have been informed of the following facts and matters, and I agree the terms below:
I am not pregnant or nursing. I have disclosed any medical conditions such as Seizures (e.g., epilepsy), fainting, hemorrhaging (Blood loss), HIV-AIDS, or any communicable diseases, as well as heart conditions or medications that thin the blood. I have informed my piercer of any conditions like diabetes that may affect the healing of the piercing.
If I have Hepatitis A, B, or C, or any other communicable disease, I have notified the piercer and have been informed of necessary procedures to ensure proper healing of my piercing.
I do not have any medical or conditions including, but not limited to, acne, scarring (keloid), eczema, contagious skin infections (impetigo), genital warts (if relevant to the requested piercing), psoriasis at the piercing site, or any open wounds or lesions at the piercing location.
I have informed the Piercer of any allergies I have related to metals, latex, gloves, anesthetics, soaps, medications, and jewelry materials. I recognize that it is not feasible for the Piercer to assess whether I may have an allergic reaction to the piercing or its associated processes, and I understand that such reactions are possible.
If have experienced any of the mentioned allergies, it is advisable to consult a GP before proceeding with the piercing, and written authorization will be required.
I have truthfully stated to the Piercer that I am over 18 years old; if I am not, I must be accompanied by a parent or legal guardian. I confirm that I am not under the influence of alcohol or illegal drugs.
To my knowledge, I do not have any physical, mental, or medical conditions that could impact my well-being as a result of my decision to get this piercing.
I acknowledge that the decision to obtain this piercing is mine alone, resulting in a permanent alteration to my appearance, and I have not been assured that the skin involved can't be restored to its pre-piercing state.
I understand that infection can occur due to inadequate hygiene related to the piercing. I have been provided with aftercare instructions and agree to adhere to them during the healing process.
I am aware that appropriate instruments and sterilization methods will be used for the piercing.

I acknowledge that this type of piercing may take between 8 weeks to 6 months, or longer, to heal. I agree to release and forever discharge the Piercer at Skin Graphics Tattoo & Piercing Studio from any and all claims, damages, or legal actions related to my piercing or the procedures and practices involved.
(Piercing Tip:) Once fully healed, there should be no more oozing, the edges of the hole will be smooth and sealed, with no redness or crusting.
